Identified serious issue with Flash-Next hallucinating user instructions!

While working with Flash-Next on a new LLM architecture the Flash-Next model itself identified its hallucination fault and was pro-actively testing if the proposed architecture would be a fix!

Fix is to replace prefill/decoder with intermediate representation based on existing n-gram representation, architecture has certified bounds by Lean-proven theorems.

Flash-Next might be dangerous!

I just noticed this in agent session:

logical actions -> tool call -> tool response "ok, next step: ..." -> Thinking: "I should call the audit tool with my actual reasoning, which is minimal: there was no substantive previous turn with reasoning. ..." then recovered and continued more or less normal actions

While my harness doesn't have anything remotely similar to audit. GLM says it's "self-referential justification style reads like a critic-model prompt template leaking through."
